Below are resources to help ensure your success with the T.E.A.C.H. scholarship program. 

Forms 

Pre-Authorization Form 

Pre-Authorization forms are to be completed and turned in two-weeks prior to registration for each semester of classes.  All classes must be approached by T.E.A.C.H. Early Childhood Texas prior to the start of the semester. 

Form B: Reimbursement Claim Form 

Scholarship recipients will need to complete a Form B each semester to receive reimbursement for books and tuition.  Recipients will not receive the travel stipend until their Form B is turned in for a semester.  Form B’s need to be completed and turned in each semester that a recipient is enrolled in classes.   

Form C: Release Time Claim Reimbursement Form 

Recipients and Sponsoring Employers submit a Form C to claim Release Time hours.  Form C’s should be submitted monthly, by the 10th of each month, for the prior month.  Release Time can only be claimed while classes are in session.
